Columbia River Health (CRH) offers services regardless of a patient’s ability to pay. CRH does not discriminate in the provision of services to an individual based upon the individual’s race, color, gender, sexual orientation, national origin, disability or religion.
All financial counseling is completely confidential and conducted in an atmosphere of dignity and respect. We strive to eliminate the confusion and stress of the financial process.
What do I need to bring to my Sliding Scale Appointment?
In order to apply, you need proof of income for every person you are claiming as part of the household. Family size includes the applicant and anyone they are financially responsible for whether living in the same dwelling or not. You can bring this into the clinic or upload a photo or scan of the documents as part of our secure online system.

Can I Apply Online?
Yes, access the application link above to complete the necessary forms. Our patient outreach team will contact you to make an appointment to verify documents and complete the application process.
What happens after I apply?
After you have completed the Sliding Scale Discount Application and provided yours and anyone in your household’s proof of income, you can expect to receive a letter notifying you of an approval, denial, or request for additional information. Often, an answer can be given as to whether you qualify at your appointment.
How much will I pay with the Sliding Scale Program?
Your eligibility and discount depend on your household income and size. Columbia River Health offers a flat fee co-pay at 4 levels. Your outreach specialist will determine for which level you qualify.
If you need assistance signing up for Medicaid or another type of health care plan, Columbia River Health can help. Our patient outreach specialists are certified by the Oregon Health Authority for Medicaid enrollment and updates. They are trained to assist in navigating the Oregon Marketplace Health Plans. Our team is ready to answer your questions and guide you through the application process.

Can I Apply Online?
Yes! To apply for OHP online, go directly to the state website, ONE.Oregon.gov and click “Apply Now.” You will need to create a new ONE account if you don’t already have one. If you need help with the application, call us! We can help!
How do I know if I am eligible?
There are many different requirements and different programs covered under OHP. OHA’s Oregon Administrative Rules for medical programs list these requirements. To qualify for OHP individuals and families must meet income and residency requirements.
How will I be notified if I’m accepted?
If choosing to apply online, the State will notify you through text, email, or mail depending on your notification preferences. If you do not receive anything after 45 days, you may call us to find out about the status of your application.
Patient’s who are uninsured or not using insurance have the right to receive a “Good Faith Estimate” explaining how much their medical care will cost. Under the law, beginning January 1, 2022 health care providers need to give patients who are not using insurance an estimate of the bill for medical items and services.
Columbia River Health ensures that our patients receive a Good Faith Estimate in accordance with the law. If at any time before or during a visit or procedure you have questions about charges, please feel free to ask for clarification.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. For questions or more information, visit www.cms.gov/nosurprises or call Columbia River Health Billing.
Columbia River Health accepts most major insurances! Please remember to bring your insurance card and a photo ID with you at every visit. If you have any questions about insurance-specific pricing and coverage, please contact your plan directly. If you don’t see your insurance, contact your insurance company or Columbia River Health to confirm eligibility.

20% cash at time of service discount
A 20% cash at time of service discount is offered if patient is paying for services in full on same day of visit and there is no insurance or other responsible party to bill.
Budget/payment plans
It is the policy of CRH that patients who cannot pay their account in full are eligible for a budget plan to allow them to pay on their accounts in installments. Please contact the CRH billing team at 541-481-7212 Ext 7.
Post-Dated Checks
CRH does not permit post-dated checks for services or devices.
Returned NSF Checks
Columbia River Health does not accept or automatically write off checks previously deposited on patient accounts and returned for “non-sufficient funds” (NSF) reasons. Patients will be assessed the NSF fee charged by the bank. Two NSF checks received from the same patient or account will result in the patient being placed in a “cash only” status for all future visits.
When you have an appointment at Columbia River Health, please be prepared to pay your co-pay or make a nominal payment ($25) toward your services.
