REQUEST MEDICAL RECORDS

To request copies of your own medical records, you may send the completed Request for Health Information (ROI) form through MyChart or fax the completed form to 541-481-2020.

You must include a copy of your photo ID in order for us to verify your identity. You have the option of receiving your records electronically via a CD, email, fax, or a physical copy. There is a fee for paper copies:
10 cents per page after the first 20 pages.

Please note that although we send your records encrypted, if you chose to have your records sent to your personal email, it may be insecure and could possibly be accessed by someone other than yourself. Columbia River Health will not be liable for any breach that may be incurred in this scenario.
