Internet Access
Use the MyChart app on your phone or access the internet and/or a computer at home or your local library.
A Private Email Address
If you do not have an email address, you can get one for free online.
MyChart Activation Code
Ask your Care Team about the different ways to activate MyChart; activation code is case sensitive.
There are two ways to sign up. Ask your Care Team for details.
No Social Security Number is needed for this setup process.
From a Smartphone: Download the MyChart - By EPIC app for your phone. Once you've downloaded the app, search for "OCHIN," then begin the set up process. Use the activation code from your Care Team, your date of birth and last 4 digits of your social security number to register. If you have trouble with your activation code, please call 541-481-7212 and we can guide you through the activation process.
From a computer: Access the MyChart page by using the link above. Use the activation code from your Care Team, your date of birth and last 4 digits of your social security number to register.
