Columbia River Health continues to be in the process of onboarding new providers.

New Patients

We will be accepting New Patients as of June 1st, 2025

To establish care as a new patient, please complete the Registration form below and contact Columbia River Health to make an appointment.

If you wish to transfer your medical records from another provider, please complete a Release of Information (below) and drop it by the clinic before your appointment so that a provider can review your record before you come in.

Existing Patients

Once a year you will be asked to resubmit your registration paperwork. To save time, you may complete this online.

To Cancel an Appointment

Please call to cancel your appointment at least 24 hours in advance. You can also cancel the appointment through MyChart.

When you miss an appointment without canceling beforehand, other patients miss out on the opportunity to take your time slot.

What to Bring with you to your Appointment

  • A List of all Medications to Each appointment
  • Your insurance card and ID
  • For children, bring immunization records
  • A list of questions you wish to discuss with your provider

Each Patient Has the Right to:

  • Be treated with dignity and respect
  • Know the names of the people serving him or her
  • Have privacy and confidentiality of his or her records
  • Receive explanations about treatment, charges, or other services
  • Receive education and counseling
  • Review his or her medical records with a provider
  • Consent to or refuse any care or treatment

Each Patient Has the Responsibility to:

  • Give an accurate medical history
  • Be sure that he or she understands the provider's recommendations or asks questions to clarify
  • Follow health advice and medical instructions
  • Respect clinic policies
  • Report any changes in his or her health
  • Keep appointments or cancel with at least 24 hours notice
