Our Bilingual Patient Outreach Specialists and Care Coordinators focus on the needs of the whole person, not just physical. We provide transportation services, connections to community resources and financial assistance.

Columbia River Health offers assistance with your transportation needs. We can connect you with one of the community transportation services or we can offer our transportation van to those patients who cannot get to or from their appointments.

As part of your Care Team, our Patient Outreach Specialists and Care Coordinators work with your Provider and other Specialists to get you the care you need. They act as liaisons to ensure that your needs are met.
From chronic disease management to health education, they will ensure you understand your options. They may call you to remind you of screenings/tests you need and referrals that you have received.

Our Patient Outreach team is certified by the Oregon Health Authority for Medicaid Enrollment or Updates and trained to assist in navigating the Oregon Marketplace Health Plan. They are available to assist you in the financial side of healthcare. Whether it is helping you apply to Medicaid or completing paperwork, they are focused on your well-being, in all aspects.
